Title / Titel Polypharmacy in Psychiatry: Clinical Practice versus Empirical Evidence

Summary / Zusammenfassung Polypharmaceutic approaches have gained increasing attention during recent years in the treatment of affective and psychotic disorders. At the same time, however, response rates under antidepressants and antipsychotics have continuously decreased, while the proportion of incomplete responders continuously increased. Thus, the question arises as to the extent to which recent changes in therapeutic effect are explainable through (1) polypharmaceutic treatment approaches, or (2) an overproportionately large number of “mild” cases where pharmacological treatment has little therapeutic effect. To address these questions we have carried out two independent studies comprising 296 inpatients and 363 outpatients whose treatment modalities were assessed in a standardized way along with other clinically relevant variables. Based on these data we searched for factors that explained the observed response characteristics in comparison to our reference study of 2,848 patients under monotherapeutic treatment with 7 different antidepressants or placebo. Additionally, pattern analysis was used to identify characteristic “treatment patterns”.
