| Summary / Zusammenfassung | Chronic right ventricular (RV) pacing is associated with deleterious effects in long-term functional and structural outcome. The risk of heart failure due to impaired ventricular pump function increases over time. The recognition of the potential harmful effects of RV pacing initiated the search for alternative ventricular pacing sites. In the acute situation left ventricular (LV) apex pacing increases pump function as compared to RV pacing. Also, some cases are reported, in which there is functional improvement and reverse remodeling during chronic LV pacing, subsequent to chronic RV pacing. In some centers chronic LV pacing is applied based on surgical preferences. Nevertheless, evidence that LV pacing results in better function and less remodeling as compared to RV pacing is insufficient, and the optimal site for chronic pacing in children has still to be identified. Aim of the multicenter study is to investigate which pacing sites for chronic pacing in children with structural normal hearts carries least adverse effects on left ventricular mechanical synchrony and function. | ||||
