| Summary / Zusammenfassung | This is a single centre clinical study in patients with axillary hyperhidrosis. Patients with excessive axillary sweating who have failed conservative therapy with Aluminiumchlorhydroxide-solution and who have decided to undergo tumescent suction curettage are recruited at the Department of Dermatology. After giving informed consent, the excess sweating will be assessed quantitatively by gravimetric measurement, which will be used as a comparative basis. The operation will then be performed in the operating theatre of the Department of Dermatology. Prior to the operation the area of excess sweating will be documented with the iodine-starch test and marked. Thereafter a tumescent local anaesthesia in the axilla will be performed using Lidocain 1% with Adrenalin, diluted with 100ml NaCl solution. A 5 mm punch biopsy will first be taken from the centre of the hyperhidrotic area. Afterwards the suction canula will be inserted trough two 0.5 cm incisions on each side of the axillary cavity and the sweat glands will be curetted and sucked off. The same procedure will be performed on both axillae. The operation site will be bandaged. To prevent infectious complications, Patients will be given Cefuroxim 250mg bid for 5 days postoperatively unless contraindicated. The first change of bandages will be performed one day after the operation. At that time point the wound will be clinically assessed (haematoma, seroma etc.) and the patients asked about potential side-effects (pain, bleeding etc.). The next appointment will be held 14 days after the operation, where the sutures will be removed and the wound assessed again clinically (haematoma, seroma etc.) and the patients asked on side-effects (pain, bleeding etc.). One month after the operation the first postoperative gravimetric measurement will be performed to assess the quantitative reduction of the sweating. Also the patient will be asked on his subjective assessment of the success of the operation (5 point scale very satisfied to unsatisfied). This procedure (gravimetric measurement and assessment of the patients satisfaction) will be repeated 6, 12 and 24 month after the operation. Additionally, 1 month and 1 year after the operation a 6mm punch-biopsy will be performed within the centre of the operated axilla. The tissue will be stained with CEA for the sweat glands and S-100 for the nerves and the density of the sweat glands and the corresponding nerves will be assessed and compared. | ||||
