Title / Titel Evaluation of flow and pulsation of the peripheral pulmonary arteries in presence of severe pulmonary regurgitation: a magnetic resonance imaging study

Summary / Zusammenfassung We sought to evaluate the flow and pulsation characteristics of the Pas by using cardiovascular magnetic resonance (CMR) in patients with pulmonary regurgitation (PR) after surgical repair of Tetralogy of Fallot (TOF), and to assess potential correlation between the Pas dimensions and pulsation and blood flow.
CMR for quantification of PR and of right ventricular dimensions was performed in 24 patients with repaired TOF and significant PR. Median age was 18 years (range1-64), weight 48.5 kg (7.7-85). Flow and cross-sectional area changes (pulsation) during cardiac cycle were measured by acquiring phase contrast cine images through the main (MPA), the right (RPA) and the left pulmonary artery (LPA).
Significant PR was present in all subjects, with a mean MPA regurgitation fraction (RF) of 41.4 +/- 8.4%. None of the patients showed Pas stenosis. Symmetrical forward flow into RPA and LPA was observed. However regurgitant flow was larger in the LPA. The LPA maximal cross-sectional area was significantly larger than the RPA area. Pulsation was similar in the RPA and in the LPA, with a cross-sectional area reduction of 42.5 +/- 9 % in the RPA and 39.1 +/- 10 % in the LPA. A positive correlation was found between RPA pulsation and RPA regurgitant flow, RPA forward flow, MPA regurgitant flow, MPA forward flow, and right ventricular volumes. No correlation was demonstrated between LPA pulsation and all these parameters.
