| Summary / Zusammenfassung | According to previous reports, the prognosis of steroid sensitive idiopathic nephrotic syndrome (SSINS) is thought to be good as renal function is not compromised and relapses cease in up to 95% of patients in puberty. However, no recent data are available concerning the impact of intensified treatment protocols for initial and subsequent relapses. A significant proportion of patients develops frequent relapses or even steroid dependency, complications that are often difficult to treat. Long-term and possibly life-long immunosuppression may be necessary and treatment failure may ensue. In addition to the lack of recent data on medical outcome of the SSINS also the psychological outcome in this disease is not well studied. An increased prevalence of behavioural problems patients (especially during steroid treatment) but also in siblings has been reported. Recently it has been speculated, that psychosocial circumstances and especially family structure may even influence medical outcome. To address the issues of medical and psychological outcome in SSINS we propose the initiation of a large, representative single-centre study. A retrospective analysis will evaluate the long-term medical outcome with respect to number of relapses, steroid-dependency and duration of treatment in 323 patients with SSINS treated during the last three decades at our centre. A second goal is to analyse the influence of and compliance with different treatment regimens (standard vs. intensified) on the long-term outcome. In addition, the prevalence of complications, adverse events and side effects will be studied. Furthermore psychological outcome will be studied in 58 patients with SSINS currently treated at our institution. By means of structured interviews and questionnaires data on patient and family reactions, child’s psychosocial adjustment and quality of life will be collected and related to medical outcome. In summary, these data will allow a comprehensive view of the current long-term outcome of SSINS and will contribute to quality control of the medical care. Integration of psychosocial variables and their impact on clinical course of SSINS will help to optimise medical care in children and families with this disorder. |
