| Summary / Zusammenfassung | Implantable cardioverter defibrillators (ICD) in children have proven to be an effective therapy for life-threatening ventricular arrhythmias. The optimal electrode and device placement for ICD therapy in children is still controversial. Limited vascular access, patient size, and growing, have led to alternative implant techniques using subcutaneous or epicardial defibrillation electrodes. With these approaches either shock delivery has not been reliable, or defibrillation thresholds have been unfavorably high. Moreover, the main risks in children are physical activity, growth-related distortion, and unfavorable position in terms of exposure to physical impact. They have resulted in a substantial number of lead fractures or dislodgement of electrodes, leading to inappropriate shocks. Aim of the study is to evaluate whether our new approach epicardial and pleural electrode insertion with abdominal or intra-thoracic device placement is safe in children. |
